WHAT ARE MARIONETTE LINES OR A DOWNTURNED MOUTH?

Marionette lines and a downturned mouth (oral commissures) are an extension of the smile lines and present as the lines or folds that form from the corner or the mouth down to the jawline. Marionette lines and a downturned mouth are often treated in conjunction with lower face toxin, lip volumisation, perioral lines and the chin.

WHAT CAUSES MARIONETTE LINES AND A DOWNTURNED MOUTH?

The marionette lines and a downturned mouth appear as we age due to a combination of volume loss in the mid-face and chin area as well as the muscle in the area (DAO) pulling down the corners of the mouth leading to a sad appearance. 


CAN FILLERS HELP MARIONETTE LINES AND A DOWNTURNED MOUTH?

When patients present in clinic with concerns around the corner of the mouth and the marionette lines, we assess the full face and often recommend mid-face support in the form of cheek contouring first. This treatment uses hyaluronic acid dermal filler to support the fat pads in the cheek area which in turn lifts the descended volume in the marionette area and indirectly treats the problem. Injecting the marionette or mouth corners with dermal filler before supporting the mid-face can lead to a bulking and an unnatural appearance. Once the mid-face has been supported the area is then reassessed and may be treated independently or in combination with chin augmentation or lower face toxin.
